  • Case Manager
    • Position:  Case Manager
      Organization:  Jewish Family & Career Services
      Reports to: Assistant Director of Programs

      FLSA Status: Exempt
      Location:  Louisville, Kentucky

      Purpose: To provide case management activities with a range of individuals and families who are economically marginalized and/or with limited English proficiency through a holistic approach that promotes stability and well-being.

      Essential Functions:

      • Manage a caseload of clients.
      • Provide concrete services such as completing eligibility forms, resource coordination, securing government assistance, etc.
      • Conduct client assessments, identify goals, establish care plans and complete quarterly reviews.
      • Network with agency and community service providers.
      • Stay current on general community resources and local/federal entitlement programs and assist clients in obtaining them.
      • Track, monitor, case note and report client progress.
      • Collect demographic and outcome data on all clients and complete reports according to schedule.
      • Provide administrative assistance to clients on issues to completing insurance forms, securing government assistance, etc., including paperwork and in-person advocacy as appropriate.
      • Serve as Information and Referral (I&R) services with agency clients.

      Secondary Duties:

      • Maintain accurate, complete and timely client records and associated notes in the mandated systems according to agency and HIPAA policies.
      • Attend staff, supervisory, team and impact area meetings.
      • Other duties as assigned.

      Supervisory Responsibility: N/A

      Travel Requirement: Considerable local travel (Louisville Metro area) required.

      Pre-Employment Requirements:  Satisfactory completion of a pre-employment background check and driver's record check. Negative TB test and drug screen required.

      Education, Training and Experience Requirements:

      • Bachelors in human services or related field preferred. Bi-lingual in Arabic or Spanish preferred.

      Performance Requirements:

      • Responsible for meeting agency, grant and other funder outcomes in a timely manner.
      • Attend all agency and department meetings.
      • Meet reporting and departmental deadlines.
      • Maintain up-to-date client case records and outcomes measurement as mandated by internal policies and procedures, and by those with contracted organizations.
      • Handle all Protected Health Information (PHI) and Personally Identifiable Information (PII) confidentially and follow agency mandates procedures regarding the security of client/agency information.
      • Conduct self in a professional and respectful manner with coworkers and community members.

      Knowledge, Skills and Abilities:

      • Ability to develop constructive, professional and cooperative working relationships with others and maintain those relationships over time.
      • Effectively utilize general and program-specific computer programs (e.g., Microsoft Office Suite, Penelope, etc.)
      • Effectively operate typical office equipment (telephone, computer, fax machine, copier, mail machine, etc.)
      • Thorough knowledge of community resources to assist participants with needs of food, shelter, transportation, childcare, financial assistance, etc.
      • Sensitivity to different cultural, socio-economic and age groups.

      Physical Requirements and Environmental Conditions: 

      • Tasks involve sitting, standing, walking, kneeling, bending, crouching, reaching and performing other physical requirements commensurate with demands of the position, specifically conducting home visits.
      • Sedentary work which involves sitting most of the time.
      • Must be able to speak (expressing or exchanging ideas by means of spoken words)
      • Must be able to hear (hearing aids acceptable)
      • Close visual acuity to perform activities such as viewing a computer terminal, excessive reading, and preparing and analyzing data and figures (corrective devices acceptable).
      • The worker is not substantially exposed to adverse environmental conditions (typical office work).

      Salary: 

      • $35,000 - $42,000

  • Case Manager | Business Planner
    • Position:  Case Manager | Business Planner
      Organization:  Jewish Family & Career Services
      Reports to: Director of JFCS Navigate
      Position Type: Full Time
      FLSA Status: Exempt
      Location:  Louisville, Kentucky

      JFCS is seeking a Case Manager | Business Planner, who will provide business development assistance and marketing training to aspiring entrepreneurs in targeted population groups to start, grow and expand their business ventures.  Primary focus will be to provide support services and strengthen skill sets through individual and group assistance with business plans, marketing strategies, industry research, financial forecasting, and operational planning. The Case  Manager | Business Planner, will serve on the JFCS Navigate team and will report to the Director of JFCS Navigate.

       

      The Case Manager | Business Planner will manage clients/entrepreneurs to become ready for business financing through preparation of microlending applications and business modeling approaches. Through these program and training initiatives, the Case Manager | Business Planner will cultivate long-term relationships with the new entrepreneurs. The Case Manager | Business Planner will develop business training programs, including classes and workshops for small business owners to help guide their pathway to economic self-sufficiency. As part of the JFCS Navigate team, the Business Advisor will help create and maintain external relationships with partner organizations and community leaders through outreach activities and support services provided.

       

      Qualifications, Competencies and Characteristics

      • Bachelor’s degree in business administration/management, communications and marketing or equivalent experience in a related field preferred, or equivalent combination of education, training, and experience.
      • Previous experience in microenterprise development or managing for-profit business units/operations.
      • Manage case load of clients, including tracking of business technical assistance, providing financial coaching, loan packaging and guidance, etc.
      • Ability to work flexible hours, including some evening and weekends. 
      • A passion for the JFCS vision, mission and values and a desire to help “Repair the World”.
      • Strong written and verbal communication skills, including the ability to make presentations and conduct engaging and interactive training programs.
      • Experience with a broad array of business topics, to include business management, promotional and brand marketing, and financial forecasting/planning.
      • Demonstrated ability to analyze business trends and data to determine potential opportunities.
      • Excellent organizational skills and ability to prioritize and manage shifting priorities.
      • Solid working knowledge of Microsoft applications.
      • Bilingual a plus.
      • Adhere to strict confidentiality of constituent information.
      • Effectively operate typical office equipment (telephone, computer, fax machine, copier, mail machine, etc.).

  • Driver
    • Position:  Driver
      Organization:  Jewish Family & Career Services
      Reports to: Homecare Director
      Position Type: Part Time
      FLSA Status: Exempt
      Location:  Louisville, Kentucky

      Purpose: Driver will transport clients (primarily elderly) who are ambulatory and wheelchair-bound to medical and social appointments, shopping, outings and errands. Those in wheelchair will require special attention in transporting. The driver will adhere to safe driving practices and will not operate a cell phone while driving.

      Essential Functions:

      • Transport clients from designated addresses to appointed destinations as per schedule.
      • Practice safe driving habits and follow all state and national safety regulations.
      • Provide stand-by assistance to clients as they embark and disembark vehicle ensuring seatbelts are worn by all clients and assisting as necessary.
      • Escort clients to and from errands and appointments as requested.
      • Document mileage, maintenance and repairs on vehicle logs.
      • Maintain vehicle readiness ensuring it is always clean, fueled and ready for use as well as identify and check for safety concerns, tire inflation, etc.
      • Maintain communication with Homecare Director regarding any changes, safety issues, critical incidents or general client concerns.
      • Excellent navigation skills and proficiency using navigation applications.
      • Assist with other tasks on behalf of Agency that involve transportation.
      • Maintain accurate, complete and timely client records handling all Protected Health Information (PHI) and Personally Identifiable Information (PII) confidentially in accordance with agency procedures regarding the security of client/agency information.
      • Other duties as assigned.

      Requirements:

      • High school graduate preferred
      • 20+ years old
      • Valid driver's license and current, clean DMV record

      Physical Requirements and Environmental Conditions:

      • Tasks involve sitting, standing, walking, kneeling, bending, crouching, reaching and performing other physical requirements commensurate with demands of the position.
      • Must have good vision (corrective lenses acceptable).
      • Must be able to hear (hearing aids acceptable).
      • Must be able to speak (express or exchange of information by means of spoken words).
      • May occasionally life and/or carry/move/push up to 300 lbs (wheelchair-bound clients).
      • Must be physically able to assist clients into and out of vehicle and navigate clients safely to and from destination.

  • Homecare Aide

      FLSA Status: Non-Exempt
      Location:  Louisville, Kentucky

      $200 Sign-On Bonus* for new applicants! Jewish Family & Career Services is currently hiring Non-Medical Homecare Aides to begin immediately. Caring and compassionate caregivers work one-on-one with older adults in Jefferson County, assisting with personal care, meal preparation and light housekeeping. Position starts at $15/hour with opportunity for paid time off and benefits. Employees are required to pass a background check and drug screen. Flexible schedule, part-time and full-time schedules available.

      *available after working 90 hours in first six weeks.

      Qualifications, Competencies and Characteristics

      • High school diploma preferred, or equivalent combination of education, training, and experience
      • Must have the ability to read, write and speak English.
      • Training provided, but previous experience working in personal care, homecare or with the elderly population preferred.
      • Valid Driver’s License.
      • Must have reliable transportation.
      • Must be 18 years of age or older.

      • Assist clients with personal care (e.g. bathing, dressing, eating, exercises, grooming, incontinence care, medication reminders, mobility and transfers, etc.)
      • Perform companionship services (e.g. transportation services, escort to appointments and events, socialization and conversation, visit neighbors and friends, provide reminders for appointments, write letters and correspondence, play games and aid with reading, stimulate mental awareness, respite, etc.)
      • Perform homemaker services
        • Light housekeeping (e.g. laundry, change linens, make beds, empty garbage, dust furniture, mop floors, vacuum, organize and clean closets)
        • Meal planning and preparation (e.g. grocery shopping, cooking and clean-up)
        • Help with errands (e.g. dry cleaning, Rx pick up, personal shopping)
      • Adhere to strict confidentiality of constituent information
      • Other duties as assigned

      Performance Requirements: 

      • Perform shifts as scheduled (e.g. arrive on time, stay for the duration).
      • Follow provided care plans for all clients exactly.
      • Stay in communication with the Homecare Director continually.
      • Attend and complete all mandatory trainings.
      • Turn in required documentation (e.g. timesheets, flow sheets, incident reports, etc.) in a timely manner.
      • Comply with all applicable KY Personal Services Agency regulations.
      • Prompt compliance to requests for documentation and screenings (e.g. TB and drug).
      • Respect confidentiality of clients and comply with confidentiality and information security guidelines.

      Knowledge, Skills and Abilities:

      • Ability to effectively communicate in English through oral and written means.
      • Sensitivity to the elderly, convalescent, and persons with disabilities.
      • Maintain composure, keep emotions in check, control anger, and avoid aggressive behavior, even in very difficult situations.
      • Be pleasant with others on the job and display a good-natured, cooperative attitude.
      • Monitor clients and client environments and keep Homecare Director informed of potential concerns or hazards.

      Physical Requirements and Environmental Conditions:

      • This position will require the worker to climb, stoop, kneeling, crouch, reach, stand, walk, push/pull, lift, grasp, and/or feel during the course of essential duties.
      • The worker is required to have visual acuity to determine accuracy, neatness and thoroughness of the work assigned and to make general observations of facilities and structures. Corrective lenses acceptable.
      • Worker must be able to express and exchange ideas by means of spoken word. Assistive devices acceptable.
      • Worker must be able to perceive the nature of sounds at normal speaking levels, with or without correction. Assistive devices acceptable.
