Purpose: Provide older adults and their family members with help in managing healthcare and daily living needs. The position will function as a guide and advocate, providing assistance and resources to help seniors support their living situation, improve quality of life, plan and manage long-term care and navigate the healthcare system.
Essential Functions:
- Manage a case load of older adult clients.
- Conduct comprehensive assessment of physical, emotional, cognitive and social needs.
- Develop a personalized care plan to address client needs and goals.
- Assist clients and their families in navigating the healthcare system and coordinating weekly scheduled activities.
- Accompany clients during medical appointments.
- Provide information and education to clients and their families about available resources.
- Coordinate and connect clients to needed resources and services.
- Manage crises and emergencies as they arise, such as falls, hospitalizations and sudden changes in status.
- Long-term care planning.
- Provide concrete services such as, completing eligibility forms, securing government assistance, financial planning, etc.
- Monitor status on a regular and consistent basis.
- Facilitate communication between the older adult, their families, healthcare provides and other service providers.
- Track, monitor, case note and report client progress.
- Networking and coordination with community service providers.
Secondary Duties:
- Maintain accurate, complete and timely client records and associated notes in the mandated systems according to agency and HIPAA policies.
- Attend staff, team and impact area meetings.

Duties and Responsibilities
JFCS is seeking a Grants Manager to work on the Advancement Team. The Manager will be required to work closely with the Program Teams, Finance Team, and Executive Team when necessary. The Grants Manager is responsible for researching, preparing, submitting, and managing grant proposals/reports that support JFCS’ goals and meet funder guidelines and criteria. JFCS has grown significantly in the past few years and much of that growth is through foundation and government grants. This individual will fully manage the grant process for those grants including managing funder relationships, engaging in compliance reporting, supporting special project initiatives, and working with other programs to meet report deadlines. This person will also be responsible for researching new grants depending on agency needs and creating a strategic pipeline of prospective grants. This position requires strong organizational skills, writing skills, data analysis, program budgeting, project management, cross collaboration and focus on outcome measurement.
Qualifications, Competencies and Characteristics
- Bachelor’s degree in relevant field and minimum of three years of experience identifying, writing, and reporting on corporate, foundation, and government grants. Experience working with federal grants is strongly preferred.

- An ability to quickly learn the variety of programs at JFCS
- Experience with database management (knowledge of Grants.gov, Intelligrants, Grantseeker preferred)

Salary: $16/hourJewish Family & Career Services is currently hiring Non-Medical Homecare Aides to begin immediately. Caring and compassionate caregivers work one-on-one with older adults in Jefferson County, assisting with personal care, meal preparation and light housekeeping. Position starts at $16/hour with opportunity for paid time off and benefits. Employees are required to pass a background check and drug screen. Flexible schedule, part-time and full-time schedules available.
Qualifications, Competencies and Characteristics
- High school diploma preferred, or equivalent combination of education, training, and experience
- Must have the ability to read, write and speak English.
- Training provided, but previous experience working in personal care, homecare or with the elderly population preferred.
- Valid Driver’s License.
- Must have reliable transportation.

Purpose: Provide older adults and their family caregivers with resources and services to promote well-being and safety for aging in place. The Older Adult Case Manager/Counselor engages in counseling and case management activities to address personal, social and environmental challenges that arise as people age and collaborates with the entire family to facilitate long-term care planning.
Essential Functions:
- Manage a case load of family caregiver and older adult clients.
- Engage in outreach, advocacy, case management and counseling activities.
- Facilitate caregiver and grief/loss support groups.
- Provide concrete services such as, completing eligibility forms, resource coordination, securing government assistance, financial planning, etc.
- Conduct comprehensive assessments, identity goals, establish care plans and quarterly review.
- Networking with community service providers.
- Track, monitor, case note and report client progress.
- Information & Referral (I&R) services with agency clients.
Secondary Duties:
- Maintain accurate, complete and timely client records and associated notes in the mandated systems according to agency and HIPAA policies.
- Attend staff, team and impact area meetings.
- Other duties, assigned.
Travel Requirement: Considerable local travel (Louisville Metro area) required during the course of business.
Pre-Employment Requirements: Satisfactory completion of a pre-employment background and drivers’ record check. Negative TB test and drug screen required.
Education, Training and Experience Requirements:
- Master’s in Social Work, CSW preferred, or equivalent experience
Performance Requirements:
- Stay current on general community resources and local/federal entitlement programs.
- Responsible for meeting agency, grant and other funder outcomes in a timely manner
- Attend all agency and department meetings, including weekly clinical meeting with peer clinical supervision.
- Meet reporting and departmental deadlines.
- Maintain up-to-date client case records and outcomes measurement as mandated by internal policies and procedures, and by those with contracted organizations.
- Handle all Protected Health Information (PHI) and Personally Identifiable Information (PII) confidentially and follow agency mandates procedures regarding the security of client/agency information.
- Conduct self in a professional and respectful manner with coworkers and community members.
Knowledge, Skills and Abilities:
- Ability to develop constructive, professional and cooperative working relationships with others and maintain those relationships over time.
- Effectively utilize general and program-specific computer programs (e.g. Microsoft Office Suite, Penelope, etc.)
- Effectively operate typical office equipment (telephone, computer, fax machine, copier, mail machine, etc.)
- Sensitivity to different cultural, socio-economic and age groups.
Physical Requirements and Environmental Conditions:
- Tasks involve sitting, standing, walking, kneeling, bending, crouching, reaching and performing other physical requirements commensurate with demands of the position, specifically conducting home visits.
- Sedentary work which involves sitting most of the time.
