      Purpose: Provide older adults and their family members with help in managing healthcare and daily living needs. The position will function as a guide and advocate, providing assistance and resources to help seniors support their living situation, improve quality of life, plan and manage long-term care and navigate the healthcare system.

      Essential Functions:

      • Manage a case load of older adult clients.
      • Conduct comprehensive assessment of physical, emotional, cognitive and social needs.
      • Develop a personalized care plan to address client needs and goals.
      • Assist clients and their families in navigating the healthcare system and coordinating weekly scheduled activities.
      • Accompany clients during medical appointments.
      • Provide information and education to clients and their families about available resources.
      • Coordinate and connect clients to needed resources and services.
      • Manage crises and emergencies as they arise, such as falls, hospitalizations and sudden changes in status.
      • Long-term care planning.
      • Provide concrete services such as, completing eligibility forms, securing government assistance, financial planning, etc.
      • Monitor status on a regular and consistent basis.
      • Facilitate communication between the older adult, their families, healthcare provides and other service providers.
      • Track, monitor, case note and report client progress.
      • Networking and coordination with community service providers.

      Secondary Duties:

      • Maintain accurate, complete and timely client records and associated notes in the mandated systems according to agency and HIPAA policies.
      • Attend staff, team and impact area meetings.
      • Other duties, assigned.

      Travel Requirement: Considerable local travel (Louisville Metro area) required during the course of business.

      Pre-Employment Requirements:  Satisfactory completion of a pre-employment background and drivers’ record check. Negative TB test and drug screen required.

      Education, Training and Experience Requirements:

      • Social work, nursing, or equivalent experience

      Performance Requirements: 

      • Stay current on general community resources and local/federal entitlement programs.
      • Responsible for meeting agency, grant and other funder outcomes in a timely manner

      Knowledge, Skills and Abilities:

      • Ability to develop constructive, professional and cooperative working relationships with others and maintain those relationships over time.
      • Effectively utilize general and program-specific computer programs (e.g. Microsoft Office Suite, Penelope, etc.)
      • Effectively operate typical office equipment (telephone, computer, fax machine, copier, mail machine, etc.)

  • Homecare Aide

      Jewish Family & Career Services is currently hiring Non-Medical Homecare Aides to begin immediately. Caring and compassionate caregivers work one-on-one with older adults in Jefferson County, assisting with personal care, meal preparation and light housekeeping. Position starts at $16/hour with opportunity for paid time off and benefits. Employees are required to pass a background check and drug screen. Flexible schedule, part-time and full-time schedules available.

      Qualifications, Competencies and Characteristics

      • High school diploma preferred, or equivalent combination of education, training, and experience
      • Must have the ability to read, write and speak English.
      • Training provided, but previous experience working in personal care, homecare or with the elderly population preferred.
      • Valid Driver’s License.
      • Must have reliable transportation.
      • Must be 18 years of age or older.
      • A passion for the JFCS vision, mission and values and a desire to help “repair the World”
      • Assist clients with personal care (e.g. bathing, dressing, eating, exercises, grooming, incontinence care, medication reminders, mobility and transfers, etc.)
      • Perform companionship services (e.g. transportation services, escort to appointments and events, socialization and conversation, visit neighbors and friends, provide reminders for appointments, write letters and correspondence, play games and aid with reading, stimulate mental awareness, respite, etc.)
      • Perform homemaker services
        • Light housekeeping (e.g. laundry, change linens, make beds, empty garbage, dust furniture, mop floors, vacuum, organize and clean closets)
        • Meal planning and preparation (e.g. grocery shopping, cooking and clean-up)
        • Help with errands (e.g. dry cleaning, Rx pick up, personal shopping)
      • Adhere to strict confidentiality of constituent information
      • Other duties as assigned

      Performance Requirements: 

      • Perform shifts as scheduled (e.g. arrive on time, stay for the duration).
      • Follow provided care plans for all clients exactly.
      • Stay in communication with the Homecare Director continually.
      • Attend and complete all mandatory trainings.
      • Turn in required documentation (e.g. timesheets, flow sheets, incident reports, etc.) in a timely manner.
      • Comply with all applicable KY Personal Services Agency regulations.
      • Prompt compliance to requests for documentation and screenings (e.g. TB and drug).
      • Respect confidentiality of clients and comply with confidentiality and information security guidelines.

      Knowledge, Skills and Abilities:

      • Ability to effectively communicate in English through oral and written means.
      • Sensitivity to the elderly, convalescent, and persons with disabilities.
      • Maintain composure, keep emotions in check, control anger, and avoid aggressive behavior, even in very difficult situations.
      • Be pleasant with others on the job and display a good-natured, cooperative attitude.
      • Monitor clients and client environments and keep Homecare Director informed of potential concerns or hazards.

      Physical Requirements and Environmental Conditions:

      • This position will require the worker to climb, stoop, kneeling, crouch, reach, stand, walk, push/pull, lift, grasp, and/or feel during the course of essential duties.
      • The worker is required to have visual acuity to determine accuracy, neatness and thoroughness of the work assigned and to make general observations of facilities and structures. Corrective lenses acceptable.
      • Worker must be able to express and exchange ideas by means of spoken word. Assistive devices acceptable.
      • Worker must be able to perceive the nature of sounds at normal speaking levels, with or without correction. Assistive devices acceptable.

      • Maintain a clinical practice of individual, couple, family and group therapy clients through face-to-face or telehealth formats.
      • Conduct evaluation, diagnosis and treatment of emotional disorders and mental illness within the scope of clinical practice.
      • Complete clinical quarterly reviews on each client and conduct planned termination as indicated by treatment plan.
      • Provide single or multi-session family life education groups.
      • Maintain up-to-date client case records and outcomes measurement as mandated by internal policies and procedures, and by those with contracted organizations.

      Duties and Responsibilities

      The Youth Outreach Coordinator will work in concert with our Career Services, Clinical Services, and Care Management teams to connect youth to JFCS and community resources.  The Youth Outreach Coordinator will also assist the agency by developing and maintaining collaborative efforts between JFCS and other youth-serving agencies in the Louisville community.

      Essential Functions:

      • Attend community-based meetings with youth-serving nonprofit partners in the Louisville community.
      • Assist the Launch program with youth-driven career and independence curriculum and program implementation.
      • Assist the Asst Director of Programs with Implementation of Youth Be Well program.
      • Provide community youth related educational programming and group facilitation.
      • Engage in community outreach to determine ongoing youth needs and services.
      • Maintain a small case management caseload of youth.
      • Serve as a youth expert on the case management team.
      • Develop and maintain a directory of resources and services for youth.
      • Attend regular team meetings regarding youth-related services.
      • Represent the agency on community committees as requested.
